Term Grading Week: The Complete Checklist

Grading week arrives the same way every term: the term exam is checked, the class record is almost done, report cards are due, and a coordinator asks for a summary you forgot existed. The work is not hard. It is a long list of small things that each have to be right, all due in the same few days.

This checklist follows DepEd Order No. 15, s. 2026, which replaced four quarters with three terms. Work through it in order. Each step depends on the one before.

The 10-second answer: Finish scoring, check the exam split (30% ST1, 30% ST2, 40% TE), confirm every learner has scores, compute with the adjusted transmutation table, copy term grades to the report card without re-rounding, add attendance and comments, then submit through your school’s official forms and keep the evidence for at least one school year.

Two weeks before: set up

  • ☐ Confirm the dates. Term exam, class record submission, report card release, and parent conference.
  • ☐ Confirm your template. Your class record and report card must follow DO 15, s. 2026: three terms, the new descriptors, and the adjusted transmutation table for SY 2026–2027. A template from last year computes last year’s rules.
  • ☐ Blueprint the term exam. Use a table of specifications so the items match what you actually taught. DO 15 expects summative tests and term exams to be teacher-made and based on the lessons taught.
  • ☐ Pilot test if you can. A few learners from another section will surface confusing items. Our guide to pilot testing an exam shows how in one period.
  • ☐ Print answer sheets. A consistent layout makes checking faster. See printable answer sheet templates.

Exam week: score and record

  • ☐ Check papers the fast way. Our comparison of methods for checking 100 papers covers answer keys, overlays, and photographing sheets.
  • ☐ Record every score the same day. A stack of unrecorded papers is a stack of future errors.
  • ☐ Run an item analysis. Find items nearly everyone missed. Those are teaching or item problems, not learner problems. See item analysis.
  • ☐ Check the exam component. Inside the Examinations component, ST1 counts 30%, ST2 30%, and the term exam 40%, each converted to its own percentage first (DO 15, Sec. 45). Adding raw scores together lets item counts decide the weighting, which is wrong.

Grading day: compute

  • ☐ Every learner has scores in every column that was given. A blank where classmates have scores counts as zero, so chase missing work before computing.
  • ☐ Weights are right for the subject. Grades 4 to 10: core subjects 20/50/30; EPP/TLE and MAPEH 20/60/20. Senior high and Grade 12 have their own weights.
  • ☐ MAPEH is two halves. Music and Arts, and PE and Health, each computed, then averaged.
  • ☐ Transmute with the adjusted table. For SY 2026–2027, an initial grade of 70.00 transmutes to 75, and the minimum reportable grade is 60 (DO 15, Sec. 48 and Annex D).
  • ☐ Spot-check against DepEd’s example. DO 15’s Science 6 example, with an initial grade of 85.8, must transmute to 88. If your template gives 87, it is using the old table. Our MATATAG grading guide walks through it.
  • ☐ Flag learners below 75. List them now for remediation next term. See a remediation plan that actually runs.

Report cards

  • ☐ Right card for the grade. Grade 1 uses the descriptive Learner’s Progress Report with the PACE Form; Grades 2 to 12 use the Learner’s Performance Report.
  • ☐ Copy term grades, do not re-round them.
  • ☐ Names and LRNs match the School Register (SF1).
  • ☐ Attendance is on the card. DO 15 requires attendance on every term report (Sec. 70). Take it from SF2.
  • ☐ Comments are specific and brief. One strength, one next step, one way home can help.
  • ☐ Check each card before printing. Our guide to filling out the report card without errors has a ten-minute check.

Attendance checks

  • ☐ Learners near twelve absences. By the end of Term 1, contact parents or guardians and plan support (Sec. 70).
  • ☐ Learners above 20% of class days absent. Under DO 15, Annex D, this means a failing grade and no credit unless justified; the School Head may grant documented exemptions. Raise these early, not at the end of the year.

Submission and records

  • ☐ Submit through official forms and systems. DO 15 requires grades to be submitted using official DepEd forms and digital systems (Sec. 61).
  • ☐ Keep the evidence. Assessment tools, rubrics, raw scores, and feedback, for at least one school year (Sec. 61).
  • ☐ Store it properly. Physical records in secure school cabinets; digital records in official DepEd systems, networks, or authorized drives.
  • ☐ Back up your working files. One copy is no copy.

After grading: close the loop

  • ☐ Write down which competencies need reteaching and put them into next term’s first-week plans. Our weekly ILAW planning routine makes that a ten-minute job.
  • ☐ File your best evidence for your IPCRF portfolio.

Where Skoolari saves the most time

The computing and copying steps are where most of the week goes and where most errors come from. Skoolari’s DepEd mode computes the MATATAG class record under DO 15, s. 2026, including the 30/30/40 exam split and the adjusted transmutation table, and reproduces DepEd’s worked examples. The report card page builds each learner’s Learner’s Performance Report from those grades, with attendance, comments, promotion status, and proposed awards, and Grade 1 teachers get the PACE Form. The Auto-Checker scores answer sheets and runs item analysis by sex. You still check the results and submit through your school’s official forms. See the plans.
